3 Steps to Initiating YOUR Pathway

Step 1

Schedule Your Initial Assessment
Self-scheduling allows you to choose a time when most convenient for you! Or call the office to get scheduled.

Step 2

Complete Your Intake Paperwork Required
You will get an invitation from Healthie to join our client portal once you have scheduled either appointment type. The intake is thorough, allow yourself 30 minutes to complete paperwork prior to your appointment.

Step 3

Complete Insurance Information Required (if applicable)
Our billing company Headway, will request additional information for our clients scheduled for initial appointments who are choosing to utilize their insurance benefits.


Important: Steps 2 and 3 must be completed before your appointment with our providers.

We believe in clear communication about fees so you know what to expect from the start. Headway will give you a break down of your estimated cost. During your welcome call, we’ll discuss the costs of our services and the insurance options available if you are unable to use Headway. We guide you through what's included.
