Getting started is straightforward

Step 1

Schedule Your Initial Evaluation

Schedule online or call Origin Mental Wellness at 614-569-7837.

• You can choose an evaluation with our mental health providers.

• The Initial Psychiatric Evaluation is always the first appointment for new patients. Choose the time that works for you.

• We offer telehealth appointments for most services for patients located in Ohio.

• Spravato® treatment must be administered in-office.

Step 2

Complete Your Intake Paperwork Required

Once scheduled, your Client Portal invitation link expires 72 hours after it is sent.

• Please create your username and password as soon as possible to activate your account.

• Once your account is activated, your portal access will remain available.

• Intake forms must be completed at least 48 hours before your appointment.

Step 3

Confirm Your Insurance Required

If you’re using insurance, please make sure you complete the insurance form accurately and upload a picture of the front/back of your insurance card.

IMPORTANT: Steps 2 and 3 must be completed before your appointment with our providers.
